The Struggles Of Immigrant Students And The Children Of...

An issue that has become very important to me is that of fair education, specifically, in regards to ELL programs (English language learners) . This issue is very important to me because I’ve seen the struggles that immigrant students/the children of immigrants face when it come to the education system. During my early school years, I grew up in a predominantly hispanic neighborhood in East Harlem, therefore the schools in the surrounding neighborhoods had many native Spanish speakers. I didn’t encounter any problems in school with language, since English was my first language, but many of my classmates has trouble. I would often see teachers get frustrated with students when they could not communicate effectively with them. Instead of†¦show more content†¦For their other classes, the assignments were amended to complement the ELL program. There was also an after school program with teachers for homework help. When I saw this very different program, I wondered why Through my bibliographic search, I was able to learn the specifics of what scholars were discoursing on in regards to this topic. Current debates began with the teachers themselves. One article found that ELL teachers were generally â€Å"unwelcoming† (Reeves 2006). Reasons for this were the concern that there’s not enough time to address all the students’ needs, too much work for teachers when students are also enrolled in non-ELL classes, and feelings of unqualification to teacher ELL students (Reeves 2006). Another area of discussion is whether ELL programs should take an inclusion approach vs exclusion (from normal school curriculum) approach. Inclusion is when students are mainstreamed for most of the day, with some specific ELL classes. If only in ELL classes, students may not have access to the mainstream education needed to succeed (Reeves 2006). One article stressed the importance of education at home as well. If English is not also spoken at home, acqui sition is slowed (Allen 2011). Pride also affects the rate of learning, students will often claim to understand, even though they d not, to avoid the perception of being ignorant (2011). How does William Shakespeare use the play to make the audience think about aspects of love through the play ‘Midsummer Nights Dream? The play Midsummer Nights Dream is written by William Shakespeare during the Elizabethan era. During the Elizabethan times comedies were popular and this play is also written as a comedy. Comedies during the Elizabethan times consisted of a specific story of lovers and ends with everyone marrying and living happily ever after again, some of these plays were also very funny e.g. Pyramas and Thisbe. The line that once stood between them is now blurred. People find that they do no harm when they commit an act that's wrong. Violence is seen in many forms today that there was ever before. Media has a large part in broadcasting violence. Violence is due to some of these issues such as easy access to weapons, the people who commit crimes have low self-worth and self esteem and religion has become a lesser and lesser part of society. In the United States they have the Second Amendment which is the right to bear arms. This means anyone in the United States can have a gun. In the States it is said that they have enough guns to arm all the adults and half of the children. Then they wonder why their children go out and shoot people at their school. If they were such a gun happy country they would not have one of the highest death rates (by gun wounds) in the world. Since the access to many forms of weapons is gained to easily it is hard to regulate who has weapons and who doesn't. Many people who do own weapons get injured or even die from their very own weapons. The Internet also makes it easily capable to make weapons such as bombs. Although the Internet can not be controlled the access to information on any subject is great. In America they have the easiest access to weapons. Most of the kids who had been found guilty of shooting their peers at school had very low self-esteem and self-worth. Most of these kids could have been helped in one way or another. Teachers, parents, and friends of these people could have seen the warning signs. When children mope around and think nothing of themselves there is something wrong. A lot of tragedies could have been averted . If a lot more influence was put on thinking higher and better of yourself many of these children would be alive or free. Also, many of these children had been taunted or teased. If we could raise awareness on this many children would be free of depression. Most of all depression is the main cause for self-worth and self-esteem. If these kids had not been taunted they would have not sought revenge. Now a days in a catholic school when asked most kids they said they did not go to church at all. If religion was a lot more reinforced would there be so much violence? People find other types of religion much more attractive. Such as cults and gangs. They find they belong more and the benefits are greater. They have false hopes in false gods. Also the lack of a solid family life results in a poor knowledge of individuality; who they are and where they belong. In conclusion the awareness for violence needs to be looked at. How many school shootings, deaths, and other teen violence acts does it take before we realize what has gone wrong. We shouldn't have to sit there and see out friends die because of these savage acts caused by a minor dispute. Everyone needs to listen up and learn. We need to avoid the useless acts of violence. Hendricks says, â€Å"that I, as a teacher, am primarily a learner, a student among students. I am perpetuating the learning process; I am still en route. And by becoming a student again, I as a teacher will look at the education process through a radically new- and uniquely personal- set of eyes. Chapter Three The Law of Activity caught my attention. Its states, â€Å"Maximum learning is always the result of maximum involvement. It was if God was saying this is you and I believe this holds true for everyone as long as the student values the activity. One example that I believe he talked about and is near and dear to my heart is evangelism. You can teach an evangelism class and you can teach how to evangelize people, but it doesn’t mean you will be successful. You have to be active and get out and witness. One area that troubles me that he may have not elaborated enough is under the Law of Teaching page 29. He talks about not labeling kids, which I agree, but, in this same section, it also talks about how they come into our church excited and energetic and we get on to them. I have worked with these kids and they need structure and discipline and a firm hand sometimes however, it should always be done with the love of Christ. Conclusion This book is not only for the experienced teacher but for the novice as well. All can and will benefit from this book. One can tell that Hendricks is very enthusiastic about teaching and helping the learner to learn especially when it comes to the spiritual aspect of teaching. I believe that the seven laws, implemented properly, will allow every teachers, pastors, and laypersons the opportunity to impact the lives of people for Kingdom of Heaven. This is a must read for anyone, especially for those in ministry.

Advantages, Disadvantages: Electronic Record Systems In the recent past there has been a significant emphasis for the need to implement Electronic Medical Record systems. However, the benefits and shortcomings of these systems need to be evaluated (Amenwert et al. 2004) and a sufficient evidence base gathered using a well structured scientific approach to justify the change and measure the organizational impact (Wager et al. 2000). This essay will address the benefits and challenges of implementing Electronic Record Systems. It will look at applications of these systems in different care settings located in different environments so as to provide a broader view of some of the issues. The roles of the various stakeholders as well as the benefits and challenges they derive from electronic health records will be outlined and a detailed evaluation of paper records and electronic records will be done. Conclusions will then be made based on this analysis. BACKGROUND In context of health care, electronic record systems can be defined as any structured record keeping system, specifically designed to support users by providing context specific information which is electronically accessible, relevant, easily transferable, accurate and complete. These records will usually hold information regarding patients care histories, non specific clinical data, medical knowledge or other health care centered information. (Coiera 2003) As the scope is very broad and Electronic Health Records (EHR) has not been well defined (Hayrinen et al. 2008a), the definition used was limited to EHR and allied systems such as Picture Archiving Systems (PACS), Pharmacy Management Systems, Computer Physician Order Entry (CPOE) amongst other systems used day to day in a clinical setting. These areas are well documented and evidence easily obtainable (Clamp Keen 2005) Role of electronic records and patient Information in population based care A study carried out by Qresearch utilized electronic records extensively with an aim to report on population trends and disease risk factors. The analysis covered a 5 year period and was able to establish that in the UK, London had the highest percentage of patients recorded as being smokers at 25% and the lowest in the South East and South West at 20%. It also established that majority of the smokers ranged from 25-34 years of age across all regions. (Qresearch- NHS 2008). The information obtained provided a platform for decision making to determine the interventions to be undertaken, where resources should be focused and to whom it should target. This kind of data mining is only possible when data is electronically stored (Bocij and chaffey, 2005) as paper records suffer from massive fragmentation (Dick et al. 1997) making it difficult to bring all the data for analysis. It has also been established that manual methods would be ineffective to deal with such large data sets and the multi dimensional analysis required (Fayyad 1996). The ability to perform such operations is a strong argument for electronic record adoption in decision support and knowledge discovery for population based interventions (Kraft et al. 2002). Furthermore, electronic records encourage consistent recording of clinical information and this becomes a key component when aggregating individual records to form datasets. Use of clinical codes allows relevant data to be derived from these unified datasets enabling reliable analysis across different sites. This in effect is key in providing specific information for health research as well as in public health planning (Ambinder 2005) Keeping in line with Pawsons model for context specific evaluation the literature was reviewed by looking at studies done in various sites represented on table 1 and later highlighting advantages and disadvantages to various stakeholders identified. General practitioners General practitioners are the initial point of contact between clinicians and patients, they play a pivotal role in ensuring that a patientÃÆ' ¯Ã‚ ¿Ã‚ ½s Electronic record is accurate, complete and current (Dick et al. 1997). As direct users of Electronic records, GPs benefit from the linkage of these record systems to other systems such as PACS or CPOE. Requests for ancillary care services can be done at a click of a button and results for tests can be accessed immediately they become available. This in effect enhances the completeness of patient records as they are updated in real time (Ambinder 2005) reducing the GPs administrative load as well as accurately populating records for future encounters (Wager et al. 2000). Additionally, GPs are able to view different aspects of a patients care at a glance, preventing duplication in testing or medication and reducing costs (Jones et al. 2008). Attention can also be drawn to areas that require GPs action through medical alerts thereb y guiding the clinician in the disbursement of care (Coiera 2003). In effect, this increases the efficiency of the GP and a study done in the Veterans Association has shown electronic records has allowed patients information to be available 100% of the time as opposed to 60% of the time reducing the amount of sessions required to resolve patient issues (Wilson 2009) this, as well as enhance patient safety through, for instance, reduced prescription errors (Kaushal et al. 2001) Unfortunately, there is a substantial learning curve and clinicians require training to use these systems (Anderson 2007). GPs may spend substantial amounts of time inputting and retrieving data from these systems at the expense of interacting with their patients (Poissant et al. 2005). As information grows exponentially there is also a fear of clinicians becoming overwhelmed due to the explosion of information (Berner 2005) and the paradigm shift in culture also presents opportunities for resistance to change (Hendy 2005). System usability is also a major concern when dealing with electronic records. Young (2006) argues that this has been a major deterrent to widespread adoption of these systems. Patients and their carers In the UK, The NHS care record service is responsible for holding Electronic records for all UK citizens centrally (NHS 2008a). There is policy in place that will see all patientsÃÆ' ¯Ã‚ ¿Ã‚ ½ summary records being held nationally and detailed records held locally within the various GP practices (NHS 2008a). This in effect will lighten the burden to the patients and their carers of having to remember the list of drugs, allergies or adverse reactions they may have encountered in the past (Jones et al. 2008).This will reduce the risk from adverse events and near misses that can occur from wrong administration of medication through prescription errors and increase the amount of confidence patients will have on their clinicians (Jones et al. 2008). Furthermore, In the UK, Patients will also be able to view their own summary care records via health space (Greenhalgh et al. 2010) and ensure their records are complete and accurate leading to better patient outcomes (Waegemann et al. 2002 ). Additionally, in emergencies, where GPs may not be readily available, rapid access to the patientÃÆ' ¯Ã‚ ¿Ã‚ ½s medical history can reduce the time taken to provide critical care (Potts et al. 2004) Ultimately, the main benefit to patients will be increased quality of care and patient safety through increased choice in their care options (Eysenbach Jadad 2001). This will largely be due to portability of these records across different care functions allowing for access by a variety of clinicians. Unlike paper records, electronic records are easily and readily accessible due to the interconnection of systems through networks making them vulnerable to unauthorized access (Berner 2005) This brings a set of legal challenges as to who has access to this information (Delpierre 2004). Under the UK data protection act of 1997 and the privacy act of 1974 in the USA, data about patients is protected by law (Koeller 2002). However, with records widely distributed and at times held by third parties adequate legislation needs to be put in place and security measures such as firewalls and audit trails need to be implemented to protect this data. This raises complexities in implementation as well as expands cost (Koeller 2002) Health service teams and clinicians Health service teams such as pharmacists alongside other health care professional benefit immensely when records are easily shareable and quickly available (Wager et al. 2000) This is information can be used to reconcile medication lists as well as provide key information to multi disciplinary health professionals in a secondary care setting where information is fragmented and spread across different care pathways (Miller Sim 2004). This will reduce clinical risk as well as save time and associated costs (Koeller 2002). Furthermore, the problem inherent in paper records of ineligible notes becomes nonexistent (Schloefell et al. 2001). Coding systems in electronic records also avoids incidents of ambiguity that may be present in free text during care episodes (Dick et al. 1997). On the other hand, there is currently no universal standard for indexing and coding clinical information (Schloefell et al. 2001) this in effect is a challenge to health teams who may have difficulty in accessing patients data across different system platforms (Schloefell et al. 2001)The lack of unanimity in creating uniform standards leads to interoperability across physicians systems, hospitals and pharmacies (Wilson 2009). This limits the amount of choices available to patients and adds unnecessary difficulty to health teams when such incidences occur. Health providers, and allied organizations In the UK health providers such as health trusts and Strategic Health Authorities derive their benefits as a result of benefits derived from patients and health care teams (Jones et al. 2008). Other gains are made from quantifying resources not utilized as a result of implementing EHRs (Byrne et al. 2010) these benefits accrue and are reflected as cost savings to these authorities. Funds realized as a result of these savings allow for better planning on both macro and micro levels with resources efficiently allocated to interventions that have greater impact on the populace (Jones et al. 2008). Furthermore, organizations concerned in medical research benefit immensely when records are electronically stored. The nature of the record allows for data to be easily derived for specific research purposes for learning and for use by other allied institutions with minimal overhead (Mathers et al. 2009). Alternatively, the cost, size and complexity of implementing these systems are prohibitive (Hendy 2005). Many health providers such as the NHS in the UK meet many of these costs. According to the Donabedian perspective, the inputs required in setting up a working electronic record system are both time and cost intensive (Koeller 2002). Infrastructure in terms of hardware and software, expertise, training and associated costs are high especially when these are scaled to a national level (Cressman et al. 2006). In the UK 105BN pounds has been allocated to the NHS for 2010/2011 fiscal year (DoH 2010) and $19BN has been allocated for the year 2011 to promote the adoption of EHR in the USA (Wilson 2009). This however is seen to be inadequate and further investments needs to be done. Comparison of Electronic and paper records physical and described as being material objects with concrete locations, attachments and marking (Bearman 1996). Data is usually handwritten and takes the forms of Narrative text in most instances forms can also be used to insert data. Data entry lacks validation and prone to errors. Conclusions The literature reviewed provides evidence that supports adoption of electronic health records. An analysis of the literature as represented on table 1 showed good evidence indicating improvement of patient safety and increase in efficiency of clinical staff as a result of implementing EHR. Evidence showing an increase in information quality was mainly qualitative and cost benefit analysis was scarce and hard to find. Future benefits and cost savings were predictive at best and were mostly attached to reduction in risk to patients and the assumed calculation of costs savings associated with this. Hayrinen et al. (2008b) stated electronic health systems are as yet not properly defined and methods to evaluate these systems are still not well developed as a result. This in effect raises major concerns, the first being the sustainability of these systems over the long term, the safety of the patients and the quality of information provided (Coiera 2009). As we move forward, systems are becoming larger and more complex and the cost to benefits have major implications to the national GDP of most countries implementing a nationwide health strategy. It is in this respect that the top down approach adopted by the NHS has been favored to the bottom up approach or sporadic approaches in other parts of the world (Cressman et al. 2006). This approach aims to reduce incompatibility in data reconciliation across systems which not only hinders the continuity of patient care but also makes deriving data in a coordinated and timely manner for use in public health highly complex. However, both systems are challenged and other arguments have been made that favor a middle out approach where local institutions drive the process but with oversight from governing bodies who set the standards and policy to guide it (Coiera 2009).

Our Marriage Our official journey began on August 2, 1997 in Las Vegas. That was our wedding day and my official entry into married life. Tim and I said, ?I do? in Clark County, Nevada. The clerk declared us 'best friends for life' in a ceremony with just the two of us. That declaration was more profound and welcomed than one any priest could have made. Our wedding cost a total of seventy-two dollars, this included the license and ceremony. I should also include the cost of what I call 'our wedding planner.' His name was Jake and he was a taxi driver, he charged us fifteen dollars and we tipped him fifteen for his extra duties, it was a special day. When we arrived in Las Vegas from Boulder our car started to over heat from the long drive through the hot desert. We decided to park it at the hotel. We hailed a cab and set off to find a place to get married. We had a deadline; our honeymoon was scheduled to begin the next day. The cab driver drove us past several different chapels giving us details about each one. After a brief survey of chapels we asked him to drop us off at the justice of the peace. The chapels just seemed too cheesy and besides, I had no real affection for Elvis. He dropped us off in front of a large, very official looking building to get the license and he directed us across the street to a small brick office wher e the ceremonies are held. I had the distinct feeling that he has provided this service a few times before in his career. I soon found myself waiting in line behind a frail elderly man with a huge smile on his face; he stood next to a young girl, she looked equally happy. The man wore a red velvet jacket and the girl wore a shimmering red dress with spaghetti straps. It was a dress that would... ...ming with life. The smell of the flowers was intense and enlivening. The breeze that was not restricted by car windows, the heat that was not reflected by a rooftop or eradicated by air conditioning, the rain that was not repelled by anything more than my poncho, I was one with all of it. As I biked past, I moo'd as loud as I could at the cows in the fields and felt happy doing it. I even occasionally rode in the van when I was tired. Today I question my fears before I give into them. The questioning usually results in nothing that I can rely on as a concrete reason for forgoing an experience. The way we chose to get married, I had up to the last minute to change my mind. No wedding guests, invitations, seating arrangements, flowers, or caterers. Today seven and a half years later, I can say my marriage is the thing that has brought me the most joy in my life.

Durango Street Reading Log for Chapters 15, 16, 17 Character: Alex Robbins Alex Robbins is a social worker. His goal is to break up the gang â€Å"the moors†, but until chapter 17 he has no success. Alex Robbins is one of the main characters in the book, because he affects the gang â€Å"the moors†. In chapter 17 he is at Rufus’ home, because â€Å"The Gassers† cut the pigtail of Rufus’ sister. When Rufus comes home Alex Robbins is talking to his mother, that he wants to talk with â€Å"the Gassers† about what happened. But when Rufus asks about what happened, Alex tries to tell him a lie. But Rufus is smart and knows that he is telling a lie. After Rufus knows what happened, he is very angry and Alex sais that he wants to solve this issue, but Rufus sais that he has no idea of gangs. I think the idea of Alex is very absurd. In this case there is no way out. He can’t speak with the police, because it will make more problems. Alex also made a very inexperienced impression, when he talked the first time to â€Å"the moors†. Everybody laughed about him and he couldn’t accomplish himself. He seemed to be much untaught. He is a little bit the opposite of Rufus, who is very smart. Finally, I think Alex really wants to reach his goal to break up â€Å"the moors†, but he has to learn a lot, how to handle these guys.

He loved to go to the stadium and just recap all his old memories from when he used to play and watch the game. Many parts of this storey the man looks back on when he used to watch and play baseball and he describes all the feelings and atmosphere that was around. He dreams back from when he was a child and pictures himself in the same stadium playing ball. Youve always struck me as a man who dreams... I think we have things in common. I think you might like to come with me. I could show you what I dream, paint you pictures, suggest what might happen. There he is asking his friend to come and dream with him back at the old stadium at night. He wants everyone to see what he can see and share his memories with many people. He later tells his friend to invite friends and his friends to invite their friends or family. Throughout the storey this man shows great leadership and care in what he does. It shows how much he loved the game of baseball. Tomorrow night would be fine. But there will be an admission charge... Fibonacci essays Leonardo Pisano Fibonacci, or more commonly known as Fibonacci, was born in Pisa, Italy in 1175. He was the son of Guilielmo Bonacci, a secretary of the Republic of Pisa. His father was only a secretary, so he was often sent to do work in Pisan trading colonies. He did this for many years until 1192. In 1192, his dad got a permanent job as the director of the Pisan trading colony in Bugia, Algeria. Sometime after 1192, Bonacci brought Fibonacci with him to Bugia. Bonnaci expected Fibonacci to become a merchant and so arranged for him in instruction of calculation techniques. One of the major themes in this involved the Hindu-Arabic numerals which had not yet been introduced into Europe. Eventually, Bonacci enlisted his son's help in carrying out business for the Pisan republic and sent him on trips to Egypt, Syria, Greece, Sicily, and Provence. Fibonacci took this grand opportunity offered by his father, to study and learn the mathematical techniques in use in these various re gions. Around 1200, Fibonacci returned to Pisa where, for at least the next twenty-five years, he worked on his own mathematical compositions. Fibonacci produced his most famous book, Liber abaci (the book of the Calculator). The book consisted of four parts, and was revised by him a quarter of a century later (in 1228). It was a thorough article on algebraic methods and problems which strongly emphasized and advocated the introduction of the Indo-Arabic numeral system, comprising the figures one to nine, and the innovation of the "zephirum" the figure zero. Caring for a Child or Young Person with Severe Illness Introduction This assignment will reflect on and critically study an incident from a clinical setting whilst using a model of reflection. This will allow me to analyse and make sense of the incident and draw conclusions relating to personal learning outcomes. The incident will be described and analysed, followed by the process of reflection using Driscolls Reflective Model (2000) as it facilitates critical thinking and in-depth reflection which will help me to accumulate learning objectives for the future. To comply with the Nursing and Midwifery Council (NMC) (2015) Code of Conduct, confidentiality will be maintained therefore the individual will be known throughout as Ben. Reflection is defined as a process of explaining and expressing from our own experiences and helps to develop and improve our skills and knowledge towards becoming professional practitioners (Jasper, 2003). I have chosen to use the Driscolls Reflective Model (2000) as a guidance as it is straightforward and encourages a clear description of the situation which will allow me to look at the experience and identify how it made me feel, asking what was good and bad, and what I can learn (Sellman and Snelling 2010). Wolverson (2000) includes this as an important process for all nurses wishing to improve their practice. What? Ben was born prematurely following an emergency caesarean section, whereby he received prolonged resuscitation and suffered severe hypoxic-ischaemic encephalopathy (HIE). According to Boxwell (2010), infants with severe encephalopathy have a 75% risk of dying with coma persisting, or progressing to brain death by 72 hours of life. There was a realisation that continuing treatment may be causing Ben harm in that it was unlikely to restore his health or relieve suffering. Boxwell (2010) further states that survivors of HIE carry an almost certain risk of poor neurological outcome. It is these times when consideration must be given to withholding and/or withdrawing treatment, subsequently re-orientating treatment to compassionate care. I was informed by my mentor that there would be a multi-disciplinary team (MDT) meeting to discuss and justify the decision to withdraw treatment. I was invited into the MDT meeting by my mentor to both witness and actively participate in the discussion if I felt confident enough. The MDT consisted of two paediatricians, a paediatric registrar, the neonatal sister, and myself, a paediatric student nurse.   The Royal College of Paediatrics and Child Health (RCPCH) (2004)   suggest that all members of the health care team need to feel part of the decision-making process in that their views should be listened to. At the time, I was hesitant to contribute due to my knowledge, understanding and experience surrounding the clinical and ethical matter. However, I was reassured that greater openness between disciplines will facilitate better understanding of individual roles and enhance the sense of responsibility (RCPCH, 2004). We considered what was legally permitted and required, but also at what was ethically appropriate. In considering quality of life (QOL)   determinations, it was important to refer back to the ethical foundation involved with surrogate decision making, which is the standard of best interest. Some professionals argued that Ben had no prior QOL on which to base a judgment. The Children Act (1989) provides an overall statutory framework for the provision of childrens welfare and services but makes no specific provision concerning withholding or withdrawing treatment (RCPCH, 2004). It does however state that the welfare of the child is paramount which is further supported by The United Nations Convention on the Rights of the Child (1989). Article 3 under this legislation states that actions affecting children must have their best interests as a primary consideration (RCPCH, 2004). The NMC (2015) framework governs the maintenance of standards of practice and professional conduct in the interests of patients, acting as a guide to ethical practice within nursing. The principle of non-maleficence is one of the hallmark principles of ethics in health care which prohibits healthcare professionals from doing any action that will result harm to the patient. Also paramount, is the goal to restore health and relieve suffering, promoting good or beneficence. In the principle of beneficence, nurses are obliged to protect, prevent harm and maintain the best interest for patients (Beauchamp Childress, 2001). Those involved needed to be conà ¯Ã‚ ¬Ã‚ dent in their ability to understand the ethical dilemmas they faced, and had to ensure they were aware of the underlying ethical principles to support their contribution to the discussion. The decision to withdraw life sustaining treatment should be made with the parents on the basis of knowledge and trust, but ultimately, the clinical team carries the responsibility for decision making, as an expression of their moral and legal duties as health care professionals. It is not uncommon for parents to feel indecisiveness, shame or guilt about the decision to palliate their neonate, particularly when the outcome of the neonates condition is uncertain (Reid et al, 2011). However, the final decision to withdraw intensive care was made with the consent from both parents, and this was clearly recorded in his clinical notes, together with a written account of the process and factors leading to the decision. So What? Parents impending the loss of their infant experience a complex emotional reaction to their situation, typically one of anticipatory grief, shock and confusion (Gardner and Dickey, 2011). They may also experience feelings of profound loss, related not only to the imminent loss of their child but also to a loss of their expectations, aspirations and role as parents (Gardner and Dickey, 2011). Parents are fundamental in the decision-making processes around neonatal palliation and as it is they who will be the most significantly affected by these decisions (Branchett and Stretton, 2012), neonatal EOL care places a particular focus on caring for parents. Developing a flexible, transparent and family-centred care plan is essential, and so that their preferences are met, parents should take a key role in this process (Williamson et al, 2008). Spence (2011) recommends that a holistic approach is taken to clarify the familys wishes, desires and needs in order to effectively advocate for infa nts. Whilst most parents wish to be involved in decisions and planning around EOL care for their baby, some may find this responsibility overwhelming (Williams et al, 2008). Despite this, we exposed the parents to a range of options which they synthesised in order to make the best decisions for their family. However, it was important for the neonatal nurse and I to realise that highly emotive situations can often cause significant deficits in parents ability to comprehend and process such information (Williams et al, 2008). As competent nurses, it is our responsibility to provide nursing care that advocates for our patients rights in life and death, showing respect and dignity towards them and the family. We advocated for Ben by protecting his rights, being attentive to his needs, ensuring comfort and protection, and by participating in the ethical discussion to ensure a collaborative perspective of ethical negotiation (Spence, 2011). The National Association of Neonatal Nurses (2015) suggests that palliative care should include comfort measures, such as kangaroo care, an ongoing assessment of pain using an appropriate pain assessment tool and written care plans to manage discomfort, pain and other distressing symptoms such as seizures using the least invasive effective route of administration. As the parents wished to be present at time of death, the neonatal nurse prepared the family for what they would observe as life-sustaining treatment was discontinued. This included informing them of gasping and other noises, colour changes, and stating that Ben may continue to breathe and have a heart rate for minutes or hours. This is an fundamental aspect of palliative care, and provides the family with the opportunity to ask questions. However, a study conducted by Ahern (2013) stated that nurses often express anxieties surrounding how to support parental grief and how to prepare them for the imminent death of their inf ant. Parental preferences were also assessed, including whom they wish present, whether they want to hold the infant, and whether they wished to participate in any rituals or memory-making activities. Although my mentor took the lead role in planning the infants EOL care, my contribution focused on memory-making activities. Although this is often nurse initiated, making memories is increasingly recognised as an aid in parental coping and grieving (Schott, Henley and Kohner, 2007). However, McGuinness, Coughlan and Power (2014) reported that rather than physical keepsakes, parents and families instead appreciated other actions and gestures that demonstrated respect for their needs, including having time alone with the infant and being encouraged and supported to provide care to their baby. I asked the parents if they would like photos to be taken, and although parents declined photography, I offered to take some to keep in the medical records in case they decided they would like them at a later date which they appreciated (Mancini et al, 2014).   Despite this, the parents were acceptant of the offer to keep items that were related to Bens care, including his wristband, blank ets and hat. Throughout planning Bens EOL care, the effectiveness of the therapeutic relationship in meeting the familys needs was achieved by showing empathy, and by doing so I obtained the individuals trust, and respect. Carl Rogers (1961) has influenced the shift from a task- to a person-centred and holistic view of nursing care, with the adoption of Rogers core conditions (Bach and Grant, 2005). Rogers identified unconditional positive regard, genuineness and empathy as necessary conditions for helping someone change effectively through a good therapeutic relationship. This was   achieved through both proficient nursing knowledge and utilising interpersonal communication skills. According to Jones (2007), there is little research in nursing literature that discusses interpersonal skills, particularly in nursing education. There is also a critique that nursing education is often removed from the realities that students experience during their clinical practice (Bach and Grant, 2005). I felt confident and assured that my interpersonal skills would bring positivity throughout a very difficult time, helping them through the grieving process. I acknowledged that both parents appreciated my forward-thinking and empathy towards the current situation. Being empathetic during this situation required my ability to be understanding not only of the parents beliefs, values and ideas but also the significance that their situation had for them and their associated feelings (Greenberg, 2007). Egan (2010) identià ¯Ã‚ ¬Ã‚ es certain non-verbal skills summarised in the acronym SOLER that can help the nurse to create the therapeutic space. I did this by sitting facing the family squarely, at a slight angle; adopting an open posture; leaning slightly forward; maintaining good eye contact, without staring and presenting a relaxed open posture. To enhance the communication through these skills, I used active-listening skills to ensure a successful interaction through techniques that facilitated the discussion. I did this by using sounds of encouragement, demonstrating that I was listening and assimilating the information provided by the parents. This was also done by summarising, paraphrasing and reflecting on the feelings and statements. Effective use of reà ¯Ã‚ ¬Ã¢â‚¬Å¡ective skills can facilitate exploration, build trust, and communicate acceptance and understanding to the individual (Balzer-Riley, 2004). Geldard and Geldard (2005) state that it is often the paralinguistic elements of speech rather than what is actually said that betray true feelings and emotions. Now What? As EOL approached, Ben was extubated on the neonatal unit and transferred to the bereavement suite whereby my mentor continued to provide one-to-one care.   I was not present throughout the final palliative care phase as I wanted to respect the familys privacy. At this point, I held emotions of helplessness, sadness and anxiety, therefore I took some time to reflect on what had happened. It is important that nurses recognise and confront their own feelings toward death so that they can assist patients and families in EOL issues (Dickinson, 2007). Nurses often experience sadness and grief when dealing with the deaths of patients, and without any support, can suffer distress (Hanna and Romana, 2007). Debriefing is a beneficial intervention designed to help nurses to explore and process their experiences. Irving and Long (2001) suggest that debriefing demonstrates a significant reduction in stress and greater use of coping strategies through discussion in a reminiscent fashion to let their feelings out. Through reflection, I have come to the realisation and understanding that patient death is an integral part of nursing practice in palliative care settings. I have recognised that support from all members of the MDT have positive implications for nursing students coping with stressors associated with patient death. Furthermore, the experience helped me learn the importance of both verbal and non-verbal communication. As an aspiring nurse, I have to continuously improve my communication skills because I shall be interacting with more varied patients in the future. I have also been able to utilise my knowledge of ethical principles in relation to withdrawing treatment, thereby integrating theory into practice. Conclusion To conclude, the care that patients receive has the direct potential to improve through reflective practice. Becoming a reflective practitioner will help me to focus upon knowledge, skill and behaviours that I will need to develop for effective clinical practice. Notwithstanding the existence of protections that limit the distribution over the World Wide Web of material that is harmful to minors, parents, educators, and industry must continue efforts to find ways to protect children from being exposed to harmful material found on the Internet.    Meanwhile, a prohibition on the distribution of material harmful to minors, combined with legitimate defenses, is currently the most effective and least restrictive means by which to satisfy the compelling interest of parents.    Such prohibition should include the following conduct: Whoever knowingly and with knowledge of the character of the material, in interstate or foreign commerce by means of the World Wide Web, makes any communication for commercial purposes that is available to any minor and that includes any material that is harmful to minors shall be fined and possibly imprisoned. Intentional repetition of this violation should accrue greater penalties.    This prohibition should not apply to carriers and other internet service providers, including:    (1) a telecommunications carrier engaged in the provision of a telecommunications service; (2) a person engaged in the business of providing an Internet access service; (3) a person engaged in the business of providing an Internet information location tool; or (4) similarly engaged in the transmission, storage, retrieval, hosting, formatting, or translation (or any combination thereof) of a communication made by another person, without selection or alteration of the content of the communication.

Philosophy of Nursing

Every nurse holds her own personal views and beliefs about nursing. These views and beliefs encompass the nurse’s personal nursing philosophy. The nurse may find that her philosophy changes as her practice continues to grow over time. My philosophy as a new graduate nurse twelve years ago is much different from my philosophy today. I attribute this life experiences and to the growth I have experienced as both a person and a nurse. My personal definition of nursing is based on the nurse being a loving, compassionate, dependable, competent, responsible, comforting and passionate person. A nurse not only cares for her patient’s physical ailments but cares for the whole person and family unit. Nurses promote both physical and emotional well-being. Nurses should be able to recognize cultural differences and advocate for patients in a non-judgmental manner. Nursing is centered on health both good and bad. Nurses often find patients in poor health and work to assist them back to their normal health state. Often times, it is not realistic for patients to be able to return to their previous state of health. In this case nurses help patients adapt to their illness and lead productive lives despite managing chronic illness. Nursing not only consists of health promotion but also of illness prevention. Health promotion includes prevention of illness and also improving patient’s overall well-being. Nola Pender is a pioneer in health promotion and as a nurse I use her Health Promotion Model as a guide in my current nursing practice. To consider one’s philosophy of nursing, one inevitably considers our beliefs about man. I believe man exists as a unique and holistic individual within a culturally diverse society. In my culture, man is expected to be self-reliant and responsible for himself and his family. Self-reliance is attained by the ability to provide self-care. Self-care is the ability to provide for one’s own basic needs. If self-care is not maintained, illness/death may occur. My culture, like many others, considers the sick worthy of assistance. As nurses, it is our responsibility to offer assistance in bringing people back to their normal state of health if possible. Nurses do this by intervening in a manner that is acceptable to patients while also recognizing that patients have certain rights to refuse the care offered to them. It is important for nurses to recognize the many different reasons patients may not accept the care they are offering. I believe the nature of nursing is rooted in commitment to public service and the undeniable desire to help those in need. Nursing is a process that requires continual research and learning.Nursing and Patient CareIn regards to nursing and patient care, my philosophy of nursing focuses on holistic, patient-centered care, as well as a caring and compassionate patient relationship. A holistic view of the patient allows the nurse to connect with patients on a relational level in which nurses get to understand the values of patients, and this kind of practice separates physician care from nursing care.â€Å"Holism involves studying and understanding the interrelationships of the bio-psyc ho-social-spiritual dimensions of the person, recognizing that the whole is greater than the sum of its parts,† (Dossey, 2010, p.14), meaning holistic nursing is not only concerned with a patient’s physical well being, but it also concerned with patient’s emotional, spiritual, and mental well being. Nurses, by nature are concerned with a patient’s comfort, for â€Å"comfort remains a substantive need throughout life and, as such, should be considered an indispensable constituent of holistic nursing care,† (Malinowski & Stamler, 2002).Patients who feel comfortable cope better with their illness and have faster rates of healing then those patients who admit to being uncomfortable  (Malinowski & Stamler, 2002) and as a nurse it is my goal to make sure my patient is physically comfortable as well as mentally and emotionally comfortable. It is my mission to make my care centered upon the patient’s needs and wishes. I believe it is important for patient’s to be informed and be active in their care, and I hope to have a collaborative relationship with my patients so that their needs and wants are met.I want my personal nursing to revolve around building a trusting and caring relationship with patients because â€Å"to establish a trusting relationship is necessary in order to see the situation from the patient’s perspective and an absolute prerequisite for acknowledging and realizing a patient’s values,† (Austgard, 2006, p.16). I value what is important to each patient, and even if his or her wishes are something I personally disagree with, it is still my moral and ethical responsibility to treat the personal needs of my patient and work for the best possible outcome.Nursing and Healthcare ProfessionalsNot only is nursing concerned with patient relationships, but also it involves relations with many other health care professionals as well, for nursing care could not be effective would not effectiv e without the help of other health care professionals. In the past, health care relationships stemmed from the hierarchal basis, and communication took place on a vertical level, but with health care delivery evolving, interdisciplinary teamwork has become a prominent aspect in patient care. As a nurse, I desire to have an open, honest, and effective relationship with other health care professionals while keeping the patient’s needs and wishes as the driving force for care.Communication is a key element in any relationship and it is essential in the health care relationships. Interprofessional collaborative practice has shown to improve patient outcomes and retention of medical staff (Wood, Flavell, Vanstolk, Bainbridge, & Nasmith, 2009), thus collaboration is something I feel is critical for my nursing practice. I would like for my relationship with other health care professionals to be one of collaboration, rather than competition.â€Å"Health professionals must be able to work in collaborative practice models such as interprofessional teams in order to ensure consistent, continuous and reliable care,† (Wood et al., 2009) and this supports the notion that interprofessional teamwork in nursing care is essential for the best patient care. I feel that effective  patient care and positive patient outcomes should be the driving force behind interprofessional health care relationships.Nursing and the Work CultureA work culture is the attitude and personality of a hospital unit, and I feel that a work culture can significantly influence patient care delivery. My preferred work culture would be one consisting of interprofessional collaboration, open communication, and positive attitudes. Teamwork is an essential component of nursing, and I would like to work on a unit in which teamwork is valued and put into practice.Communication is critical for a positive work culture, and I personally would like a culture in which the opinion of the staff is encou raged and valued. It is my philosophy that a positive work culture will positively affect patient moral, and this contributes to better patient outcomes. Nursing and the Environment and SocietyAs a nurse, I feel as though my responsibility to the health and safety of others goes farther than the hospital of clinic I work in. â€Å"Nursing has a disciplinary goal to contribute to the health of individuals and the overall health of society,† (McCurry, Revell & Roy, 2009) and I believe nurses are obligated to promote health in their communities and nationwide. Since my philosophy of nursing is more than a profession and that is something that is focused on the commitment to public service, it would be unethical for me to ignore the health care needs of my community.As a nurse, it is very important that I aid in addressing the healthcare needs of my personal community and environment as well as society as a whole. I feel as though it is my duty and responsibility to take an activ e role in healthcare issues among the country, including issues such as smoking cessation and primary prevention of health care. I personally would like to be a resource for my environment and take a role in health promotion throughout society.Nurses across the nation should take and active role in promoting health across the nation in working for the safety of all individuals. Not only do I feel as though it is important to focus on my community, but I also feel responsible for helping those who have limited healthcare access, especially those in underprivileged areas of the world. I desire to take part in medical trips  oversees to promote health education and wellness in areas where this information is limited.Vision of Personal Nursing PracticeMy vision for nursing practice focuses on what is doing what is most beneficial to my patient. I hope to build caring, trusting relationships with my patients as well as play a positive role in their health outcome. No two patients will ever be the same, and my vision is to individualize care for each of my patients so their personal, emotional, and physical needs can be met. I always want to value each patient as an individual and respect his or her needs and dignity.My goal is to empower each patient to be active in their care in hopes of improving patient outcomes. Personally, I would like to continue to practice nursing care that is congruent with the best evidence to date, and continue to research and discover better ways of doing things.
